Get started with Lark Forms

I. Intro
Lark Forms is a form creation tool that is integrated with Base, enabling you to customize forms, surveys, or questionnaires according to your specific needs. It features a variety of question types, diverse themes, and powerful data analysis capabilities.

II. Steps
Step 1: Create and edit form
Create a form
On the Lark Forms homepage, click Create Form.
Select a blank form or choose from one of the many templates available.

Add questions
Click Add a Question and select a type. You can choose from Open Question, Single Select, Multiple Choice, Rating, NPS, Location, Date, Person, Upload File, and Number.

Step 2: Configure form settings
Send reminders
You can schedule reminders to be sent to specified recipients or send reminders to only recipients who have not yet filled out the form.
  • Scheduled reminders: Click the Settings icon, turn on Schedule reminder, and set the time, frequency, and recipients.

  • Send reminders to those who've not filled out the form: Click Publish > Remind Them and click Send.

Select a theme
Click the Palette icon to change the background, layout, and theme. You can upload your own image or choose from one of the preset themes.

Limit submissions
Click the Settings icon and turn on Submission limits as needed.
Set the start time, frequency, and number of submissions allowed. For example, you can limit the number of submissions to 1 per week.
Allow anonymity and require login
Click the Settings icon and turn on Submit anonymously and Require login as needed.
  • Submit anonymously: Respondents' personal information, such as profile and name, will not be collected. This may be useful for collecting feedback anonymously.
  • Require login: Users must log in to Lark before they can complete the form. This may be useful for registrations or activities where following up with the respondents is necessary.

Step 3: Preview and share the form
Preview form
Click the Preview icon in the upper-right corner to preview the form. You can see how the form will look on both phones and desktops.

Share the form
Click Publish in the upper-right corner to share the form. You can share it through a link or QR code or by inviting respondents. To stop accepting new submissions, click the Settings icon in the upper-right corner of the form and turn on Pause Collection.

Step 4: Manage collected data
View and analyze data
Click Analytics in the upper-right corner of the form or right-click the form on the Lark Forms homepage, and select Search record.
You will be taken to an automatically generated dashboard that has converted the form results into intuitive charts and graphs.

Export data
Form responses are recorded in a base. Click the ··· More icon in the upper-right corner of the base, then select Export > Excel/CSV file to export the data.
